Fallon Health brings new option for aging in place to Southeastern Massachusetts    

Summit ElderCare® program offers compassionate, community-based care for older adults and their caregivers  

Worcester, Mass., Sept. 22, 2025— Fallon Health is proud to announce the expansion of its Summit ElderCare® program to Southeastern Massachusetts, offering a new option for older adults and their caregivers in Bristol, Plymouth, and parts of Barnstable counties. This marks the first time that any Program of All-Inclusive Care for the Elderly (PACE) will be available to eligible individuals in parts of this region, bringing a trusted, community-based alternative to nursing home care that supports aging in place with dignity and comprehensive support. 

The not-for-profit health care services organization recently opened a new Summit ElderCare PACE Center in Dartmouth, located at 491 Faunce Corner Road. The Center serves as a central hub for participant care, featuring clinical space alongside areas for meals, activities, and social engagement. Participants can also receive care in their homes or communities.

Recognized as one of the largest PACE organizations in the country by the National PACE Association, Fallon Health has been offering its Massachusetts program, Summit ElderCare, for 30 years. The organization continues to broaden its reach while remaining committed to delivering high-quality, coordinated care to older adults.
