Navicare nurses

NaviCare is a Medicare Advantage Plan and Senior Care Option program which provides coordinated medical, prescription and support care for seniors who are 65 or older. 

NaviCare provides all services covered by Medicare and MassHealth, plus other benefits that a member may need.

Summit ElderCare nurses

Summit ElderCare is a complete medical, insurance and social support program for elderly people who qualify for nursing home care. It is a Program of All-Inclusive Care for the Elderly (PACE).

  • Use all aspects of nursing skills – including home care, triage, med management, and care planning.

Complex Case Management nurses

Our Case Management Department at Fallon Health is responsible for all outpatient care/case management functions to ensure that Fallon members receive high-quality, cost-effective care. They perform an advocacy role that helps to direct the member and caregivers through the complex health care system.

As a Case Management nurse, you will:

  • Proactively identify and assess high-risk members with complex conditions.
  • Develop and implement individualized coordinated care plans in collaboration with the member's PCP and specialists.
  • Assist members in attaining or maintaining optimal functional status.
  • Participate in innovative clinical programs that promote member independence in the least restrictive setting.
  • Work independently and collaboratively with the health care team to ensure that members receive quality, cost-appropriate care.
  • Feel personal and professional job satisfaction by making sure our chronically ill members are getting the care they need.
  • Enjoy flexible work arrangements and telecommuting options.

Qualifications:  Graduate from an accredited school of nursing; Active, unrestricted R.N. license in Massachusetts; Experience managing chronically ill and catastrophic high-risk patients. Home health care experience and case management experience in managed care a plus.

Utilization Management nurses

Nurses in the Utilization Management team take a multi-disciplinary approach to managing complex medical and psychosocial needs of Fallon Health members and their families.

As a Utilization Management nurse, you will:

  • Organize, coordinate, monitor, evaluate and modify plans of care and service requests.
  • Advocate for members to receive the best quality of care in the least restrictive setting.
  • Work closely in with an interdisciplinary team of nurses, medical directors, contracted facilities and hospital case managers. 
  • Enjoy flexible work arrangements and telecommuting options.

Qualifications:  3-5 years’ experience as an RN in a clinical setting; One year experience as a case manager in a payer or facility preferred.
