As a Program of All-Inclusive Care for the Elderly (PACE) we provide health care and other support to adults over 55, as an alternative to a nursing home.

We may be a good fit for anyone who:

  • Needs help with daily activities
  • Has skilled needs, including nursing, physical therapy or occupational therapy
  • Could live safely in the community, with more services
