Community Care is an affordable health plan available in Middlesex County, parts of Norfolk and Worcester counties, and in one town in Bristol County (Mansfield).

What do I get with Community Care?

Fallon Health’s Community Care plans include low monthly premiums and copayments for covered medical services and prescription drugs. Plus, you’ll get extras like:

  • Free routine annual eye exams for adults and children
  • The most flexible fitness benefit available in Massachusetts, reimbursing up to $150 annually for participating in healthy activities such as streaming fitness programs, Peloton subscriptions, school and town sports, gym memberships, dance and fitness classes, home fitness equipment, WW (WeightWatchers® reimagined), aerobics, Pilates, yoga classes, ski lift tickets and much more.
  • Healthy Health Plan, a program that supports members (subscriber and spouse age 18 and older) in becoming, and staying, healthy. Our plans also cover certain telehealth visits with in-network providers who offer telehealth services, including primary care, specialty, and behavioral health visits.
  • Sapphire365, a web-based tool to help you shop for the right health care providers for you based on cost and location

Get help paying for your health insurance

Depending on your income and your household size, you may qualify to get help paying for your health insurance (subsidy). Don’t think you qualify for financial help? You might be surprised. To know if you qualify for a subsidy, please apply on the Massachusetts Health Connector website.

Looking for insurance for your small business?

If you have a small business with 1 to 50 qualifying employees and your business is located in the Community Care service area, you can purchase a Community Care plan for your employees and their families. 

  • Small businesses seeking to cover 1 to 5 employees must enroll through the Massachusetts Health Connector.
  • Small businesses with 6 to 50 employees can enroll through the Massachusetts Health Connector (where they may be eligible to receive tax credits to help offset employers’ costs) or directly through us.
