How to register for the provider portal and ProAuth

Provider portal registration

You must register before gaining access to the provider portal. To register for an account, reach out to your organization's super user, and they will create an account for you. If you do not know who your organization’s super user is, email AskFCHP@fallonhealth.org.

Out-of-network and vendor providers (e.g., behavioral health providers contracted through Carelon) will not be granted access.

About super users

• Each provider group (defined by tax identification number), must designate at least one individual to be the super user to manage portal access for their organization.
• Super users serve as their provider group's main contact for portal access and are responsible for setting up and maintaining user accounts and roles for members of their organization.

Register via online form or by emailing a completed PDF form to askfchp@fallonhealth.org.
